RNACCa - Registered Nurse Ambulatory Care Coordinator's Association
RNACCa was founded by nurses working as Care Coordinators in Ambulatory physician practices.  RNACCa nurses: aid chronically ill patients in obtaining community resources, perform home visits to assess safety, provide support and education on many chronic diseases, anticipate future needs of patients while holding the patient accountable for their health care.  RNACCa  is focused on providing high-quality service and education to assist others in developing their care teams. We are dedicated to an interdisciplinary approach to patient centered medical care.  RNACCa is currently aiding in the development a population health certification program.  We strive to enhance the enrichment of RN Ambulatory Care Coordinators and other population health focused team members.  Associate members include but may not be limited to: social workers, pharmacists, certified diabetic educators, licensed dietitians, physicians, case managers and other nursing specialties.  RNACCa bases their practices on evidence based research and we hope to aid in evidence based research in the field of Ambulatory Care Coordination in the future.

Topic:  The Healing Center of Greater Cincinnati
The Healing Center provides help to residents of Greater Cincinnati when challenges invade their lives—whether sudden financial issues, job loss, divorce, loss of loved ones or just being overwhelmed caring for family. As a not-for-profit organization, the Healing Center provides assistance regardless of economic circumstances, ethnic origin, or religious affiliation.
